Forever Care Disability Services provides Supported Independent Living (SIL), accommodation, and emergency accommodation supports for NDIS participants who require a safe, structured, and supportive living environment.

Our supports are designed to promote independence while ensuring participants receive the level of assistance they need to live safely and with dignity.

Supported Independent Living (SIL)

Supported Independent Living is for participants who require ongoing support to live independently in a shared or individual living arrangement.

Our SIL supports focus on:
Building independence and daily living skills
24/7 or rostered support tailored to participant needs
Consistent support workers to provide stability and routine
Support with personal care, daily living, and household tasks
Mental health and psychosocial support where required
Behaviour-aware and trauma-informed support approaches

Who Our Accommodation Supports Are For

Our accommodation and SIL supports are suitable for participants who:
Require Supported Independent Living under their NDIS plan
Have complex mental health or behavioural support needs
Need structured environments and consistent routines
Are transitioning from hospital, family care, or other living arrangements
Require short, medium, or long-term accommodation solutions

Emergency Accommodation Support

We understand that housing crises can occur unexpectedly. Our emergency accommodation support focuses on providing immediate safety, stability, and continuity of care.

Emergency accommodation support may include:
Short-notice accommodation support (subject to availability)
Increased staffing and supervision during crisis periods
Support during mental health or behavioural escalation
Coordination with families, support coordinators, hospitals, and clinical teams
Stabilisation-focused support with clear next steps

Support Provided in SIL & Accommodation Settings

Participants in SIL and accommodation settings may receive:
Daily living and personal care support
Support with routines, meals, and household tasks
Mental health and psychosocial support
Behaviour support aligned with approved Behaviour Support Plans
Medication prompting and monitoring as per care plans
Community participation and skill development support
